The Nurse Practitioner or Physician Assistant will provide high-quality primary care and support services to individuals and families within the local community. This position is a full-time, day-shift role, where the provider would work 3-10 hour shifts from 8:00am-7:00pm. The provider will work collaboratively with physicians, nurses, and other healthcare professionals to deliver patient-centered care in a community health setting. Responsibilities include conducting comprehensive health assessments, diagnosing and managing acute and chronic conditions, and developing individualized treatment plans.

The NP/PA will provide preventive care services such as immunizations, screenings, and health education to promote wellness and disease prevention. This role also involves managing follow-up care, coordinating referrals to specialists when necessary, and ensuring continuity of care for patients. The provider will maintain accurate and timely documentation in electronic health records, adhere to clinical protocols and regulatory requirements, and participate in quality improvement initiatives.

Strong communication skills are essential for educating patients about their health, collaborating with interdisciplinary teams, and fostering trust within the community. The NP/PA will also play a key role in addressing social determinants of health by connecting patients to appropriate resources and support services. This position requires a commitment to compassionate, culturally competent care and the ability to work effectively in a fast-paced environment while prioritizing patient needs.

This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines.

Education:

Required:

RN with a master's degree in nursing certification in family and/or adult nurse practitioner. Graduate of a nationally accredited school of nursing

Experience:

Required:

Previous experience in a FQHC strongly preferred Position Requirement(s):
License/Certification/Registration

Required:

Current licensure to practice nursing in the State of Tennessee. Eligibility to sit for certification in area of specialty The Nurse Licensure Compact will not change how to obtain or renew a Tennessee license. However, the Tennessee nursing license will be a single state license for Tennessee Residents or non-compact state residents. Tennessee licensure or multistate licensure from a compact state must be obtained within three months of hire for non-Tennessee residents.
