Requirements:
Current state license as a Registered Nurse
HCHB and Oasis experience
1-2 years of experience as a Registered Nurse in a clinical care setting
2+ years of home health experience
Proof of current CPR certification
Current driver's license
Reliable transportation and insurance
Roles & Responsibilities:
Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs.
Develop, implement, document, and evaluate the Plan of Care (POC) for quality and continuity of care.
Educate patients and families regarding care, diet, medications, and home safety.
Supervise and provide clinical direction to home health aides and nursing assistants.
Monitor and manage pain through appropriate techniques.
Serve as a liaison among patients, families, team members, and healthcare professionals.
