Administer skilled nursing care for clients of all ages in their place of residence. Assess home health and/or hospice patients to identify physical, psychosocial, and environmental needs. Implement, develop, document, and evaluate the Plan of Care (POC) to ensure continuity of care. Educate patients and their families regarding care, diet, medications, and home safety. Provide clinical direction to the HHA/STNA/LVN for quality service delivery.

Monitor assigned cases to ensure compliance with third-party payer requirements. Perform on-call responsibilities and manage effective communication with patients, families, and team members.

Shift: M-F until shift is complete, on-call required if requested. Local travel required.

• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
• Current state license as a Registered Nurse.
• CHPN certification preferred.
• Proof of current CPR certification.

• 1-2 years experience as a Registered Nurse in a clinical care setting.
