Humana Inc. is seeking a Center Well Home Health LPN to provide one-on-one in-home nursing care under RN supervision. You will implement care plans, educate patients and families, and document interventions to support recovery and independence.
Requirements include an active LPN license in the practicing state, a valid driver’s license with insurance, current CPR, and at least two years of clinical LPN/LVN experience. Home Health experience preferred.
