Family Practice​/Primary Care Nurse Practitioner

Our mission is to help patients regain strength and return home safely, while avoiding unnecessary complications and hospital readmissions.

We are seeking a Nurse Practitioner or Physician Assistant to join our team caring for patients in a skilled nursing and long-term care facility in Chattanooga, TN, Rossville, GA, and Fort Oglethorpe GA.

This position is PRN.

Call coverage is provided by a national telehealth group, with the NP/PA available for back-up and oversight of any hospital transfers.

Experience in post-acute or long-term care (PA/LTC) is preferred, but providers with other relevant experience are encouraged to apply.
