Night Shift Licensed Practical Nurse

Signature HealthCARE seeks a Night Shift Licensed Practical Nurse to provide resident-centered care in our long-term and post-acute community. As an LPN, you will deliver overnight clinical care, including medication administration, treatments, assessments, and documentation, while promoting dignity, safety, and comfort. You'll collaborate with an interdisciplinary team, support CNAs, and uphold our faith-based, mission-driven culture of compassion, teamwork, and continuous improvement.

Responsibilities

• Provide nightly direct nursing care to long-term and post-acute residents per care plans
• Administer medications and treatments, including wound care and monitoring responses
• Document assessments, interventions, and outcomes accurately in EHRCollaborate with RNs, CNAs, therapists, and providers to support care goals
• Monitor changes in resident condition and promptly report to supervising nurse or provider
• Support resident comfort, dignity, and safety in a person-centered care model
• Supervise and support CNAs on night shift to ensure quality care delivery
• Follow infection control, fall prevention, and facility safety protocols
• Communicate effectively with residents, families, and interdisciplinary team members
• Participate in ongoing training, quality improvement, and mission-driven initiatives
