Are you passionate about helping people maintain stable housing while building independence and community connection? The AIM Center is seeking a Permanent Supportive Housing Case Manager who believes in person‑centered, trauma‑informed support and understands that recovery happens through relationships, structure, and opportunity. This role goes beyond traditional case management — you’ll work alongside residents in their homes and community, helping them strengthen life skills, access resources, and stay successfully housed.

- Support residents through assessment, planning, implementation, coordination, monitoring and evaluation
- Collaborate with AIM Center program staff and housing residents to develop a service plan addressing housing needs for applicants, with evaluation and updating every six months
- Monitor progress or lack of progress toward housing goals set forth in individual service plans and assist in revisions as necessary
- Complete all required documentation that accurately and fully describes the resident’s progress toward their goals and support activities
- Notify AIM Center clubhouse support staff, case managers, and other identified support persons of resident issues as appropriate
- Serve as liaison between residents and housing management regarding complaints and grievances
- Teach appropriate social behavior and other skills to help promote a person’s independence while in the community.
- Assist people in advocating for themselves and learning more about their rights.
- Determine the need for and provide or arrange for life skills instruction either on or off‑site.
- Assist in scheduling and facilitating meetings for potential roommates.
- Assist with move‑in processes.
- Plan and attend monthly residential socials.
- Create and distribute monthly calendar working side by side with a resident member.
- Distribute/inform residents of specific AIM Center unit opportunities including classes, socials, and wellness & recovery programs.
- Our case management approach is centered on empowering our tenants. We focus on tenant goals, preferences, and strengths to guide our work. The services we provide are collaborative, not prescriptive. Cultural humility and respect must guide all our interactions with tenants.
- Promote and support residents’ participation in educational, employment, support, recreational, and enrichment opportunities available through the AIM Center and the broader community, including Clubhouse social events.
- Facilitate meaningful community engagement opportunities and support residents in identifying and exploring their interests to promote sustained community involvement.
- Support and empower residents in organizing peer support groups, community activities, structured programs, and guest speaker presentations that promote engagement and connection.
- Build positive relationships within the community including community members, law enforcement, housing and social service agencies.
- Plan and attend monthly resident meetings.
- Attend neighborhood and community meetings as appropriate.
- Provide transportation for housing residents to community events and activities, including those hosted at the AIM Center.
- Participate in a rotating, on‑call schedule for resident and property related emergencies.
- Maintain necessary competence to complete job duties as described, including necessary training and certifications as assigned.
- Assist in leadership in housing and administrative functions as needed.
- Assist in other job duties as assigned in the Housing Resource Center.
- Participate in Clubhouse social and holiday staff rotation coverage.
- Bachelor’s degree in social work, preferred, or a social services degree in a directly related field.
