Social Worker, PRN
Assist patients who have complex psychosocial needs, and requiring assistance with eligibility determination for social programs and funding sources as well as qualification for community assistance from a variety of special funds and agencies. Offer short-term counseling and crisis intervention to patients and families with psychosocial needs and coordinate and facilitate the development of a safe discharge plan of care for high-risk patient populations.
Education:
Required:
Graduate from an accredited Social Work Program with Bachelors in Social Work. Preferred: MSW
Experience:
Required:
Clinical rotation experience can be taken into consideration in lieu of experience if it is applicable to the job being applied for. Preferred:
Experience in hospital social work. Knowledge of managed care practices, community resources, regulatory and insurance reimbursement issues preferred.
