The Hamilton County Supportive Housing Team Leader at Helen Ross McNabb Center oversees a multidisciplinary team providing housing-focused behavioral health services to vulnerable adults. Responsibilities
- Lead a supportive housing team serving adults with behavioral health needs in Hamilton County
- Coordinate intake, assessment, housing placement, and ongoing case management services
- Develop and monitor individualized service and housing stability plans
- Provide clinical oversight, coaching, and supervision to case managers and support staff
- Collaborate with landlords, community agencies, and internal clinical teams to secure and sustain housing
- Ensure compliance with agency policies, grants, and state and federal regulations
- Track outcomes, complete documentation, and prepare reports in a timely and accurate manner
- Facilitate crisis intervention, problem-solving, and linkage to community resources
- Promote trauma-informed, recovery-oriented, and client-centered care
- Support program development, quality improvement, and staff training initiatives
