Registered Nurse (RN) Home Health Admissions (SOC) – Full‑time position in North Chelmsford, Massachusetts.

• Complete OASIS Start of Care (SOC) assessments for adult and geriatric patients.
• Accurately document observations, interventions, and evaluations pertaining to client care management using a touch pad tablet.

• Current RN license in Massachusetts.
• Graduated from an accredited nursing program.
• Medicare Certified Home Health experience required.
