Community Teamwork Inc is seeking a Diversion Case Manager to provide compassionate case management and advocacy. You’ll help participants overcome barriers, assess referrals, and connect them with housing resources in the Greater Lowell and Merrimack Valley region.
Responsibilities include maintaining records, developing service plans, and coordinating with local homeless service providers to move clients from shelters to permanent housing while upholding CTI values of equity and integrity.
