Incompass Human Services is seeking a Family Support Specialist to serve clients aged 3–25 under the Medically Complex contract in the North East Region. You will maintain a caseload and provide active case management to help families access services and navigate the system.
You will collaborate with DDS Service Coordinators to implement individualized goals for each person, ensuring coordinated supports and effective communication with families and providers.
