Job SummaryThe staff nurse is a registered nurse who is responsible for assuring competent, compassionate nursing care for specific patients and families. The staff nurse is responsible and accountable for the overall nursing care management of patients who are living with HIV or at risk of HIV in the Chelsea Adult Medicine Department. They also serve as a liaison to the urgent care and community programs to accept and navigate referrals to care.

As an integral member of the outpatient team, the Staff Nurse provides competent, compassionate nursing care based on scientific principles and the use of the nursing process, and they oversee staff who perform case management duties.
Clinical Care- Triage and assess patient needs via telephone and during clinic visits.
- Provide initial patient assessment and evaluation.
- Provide injections and vaccines.
- Provide education and follow up for patients on antiretroviral therapy to treat HIV.
- Provide outreach and follow up for high-risk patients.
- Document relevant patient information in accordance with Minimum Expectations for Documentation.
- Communicate the Plan of Care and other pertinent information to/from other health team members.
- Collaborates with other professionals, and directs nonprofessional nursing personnel in maintaining recognized standards.
- Appropriately utilizes nursing resources.
- Utilizes resources efficiently to facilitate optimal patient care.
- Interprets departmental policies and procedures to nurses and other health team members.
- Participates in daily operational activities necessary for a safe patient/staff environment.
- Serve as a case manager and coordinate ancillary services for outpatients in need of VNA services, concrete resources, transportation concerns
- Serve as a liaison between patients and other members of the treatment team.
- Participate in social service rounds and other interdisciplinary meetings.
- Identify, facilitate, and evaluate outcomes of nursing care for an individual patient or group of patients, utilizing the nursing process.
- Serve as a resource and oversee the operational flow of the HIV clinical case management staff.
- Maintain records for and supervise paperwork collection and data entry to ensure patients are eligible for grant-funded programming and that our records are appropriately submitted to grant-funding organizations
- Participate in quality assurance activities.
- Teaches and directs all nursing personnel for whom she/he is responsible.
- Participate in orientation of new permanent and temporary staff members.
- Provides input to the Nurse Manager and Provider regarding program needs.
- Maintains and updates clinical knowledge and skills based on current nursing and health care practices.
- Adheres to requirements for mandatory continuing education and certification.
- Identifies own learning needs and goals and develops a plan to meet them
- Maintains and develops professional practice through continuing educational programs and appropriate learning experiences
- Contributes to relevant committees, professional meetings, continuing educational and/or formal academic programs.
Must have current license as a Registered Nurse in the Commonwealth of Massachusetts
