RoleWe are seeking an experienced Hospice RN Case Manager to provide care to patients in a mix of long‑term care facilities and homes.
LocationCoverage will be in Chelsea, Winthrop, Everett, MA and surrounding areas.
QualificationsRequires a minimum of 1 year of Hospice RN experience with a Medicare‑certified agency.
Responsibilities- Provide quality nursing care to identified Hospice patients.
- Perform direct patient care, coordinate services, and advocate for patients.
- Engage with patients, families or significant others, and the community.
- Comfortable handling end‑of‑life issues, death, and dying.
Full‑time, Monday‑Friday. No weekends, holidays or on‑call required.
