Minimum of two years  experience as a primary care provider preferred

• Direct Patient Care — Conducts comprehensive assessments, diagnose conditions, develop plans, and prescribe medications for patients across the lifespan. Diagnoses and treats acute and chronic disease using evidence-based guidelines. Provides preventive care, health education and counseling to individuals and families. Ensures timely and accurate documentation, including direct patient encounters and asynchronous care. Participates in coverage for clinic colleagues per departmental guidance.
  
  Participates in after-hours call coverage per departmental guidance.
• Multidisciplinary Collaboration — Participates in multidisciplinary meetings, care reviews and care planning. Coordinates care with all members of the delivery team. Contributes to the development and improvement of population-based care.
• Population Health Management — Utilizes data and risk stratification tools to prioritize patients at greatest risk, prioritizing outreach. Engages in quality improvement focused on health outcomes, cost effectiveness and cost reduction. Supports transitions of care and reduces readmissions and ED use.
• Citizenship — Demonstrates Multi Care values of compassion, stewardship, kindness, joy, collaboration and respect. Adheres to Multi Care policies and processes.

• Active Washington State Nurse Practitioner or Physician Assistant license
• DEA and state prescriptive authority
• Current BLS for Healthcare Providers certification by the American Heart Association
