Qualifications/Educational Requirements

  • Graduate of an accredited High School or equivalent preferred
  • One year of work experience
  • Ability to read and write English and follow written and oral instructions
  • At least 18 years of age
  • Ability to work positively and favorably with clients, families and staff
  • Demonstrate compassion, responsibility and cheerful attitude
  • Possess a current physical examination record (within 12 months) and provide annually
  • Possess a current PPD or Chest X-ray as appropriate (within 12 months old) and provide PPD annually, Chest X-ray every 5 years
  • To be eligible to transport clients in the client’s car, must be a licensed driver with a good driving record; to transport in one’s own car, must also have auto insurance in accordance with state requirements and an auto in good repair
  • Clear background check per Agency policy
Responsibilities/Essential functions
  • Provides companionship - emotional support, socialization, supervision, reading, game playing, monitors safety - bed rails, fall risk per service plan.
  • Provides homemaking duties such as light housekeeping, laundry, meal prep, errands.
  • Observes and reports any changes in client, safety hazards, client or employee complaints.
  • Attends client requests promptly.
  • Promotes personal safety and a safe environment for clients by observing infection control practices.
  • Documents care/services provided and pertinent observations. Submits documentation per Agency requirements.
  • Treats clients and families with respect.
  • Functions within limits of individual assignment.
  • Communicates effectively with the team and attends orientation and staff meetings as required as well as completing 13 hours of annual in-services.
  • Maintains client confidentiality, adhering to HIPAA requirements and agency policy and procedures.
  • Performs other related duties and responsibilities as deemed necessary.
  • Compliant with company vision, mission, values. Displays behavior complaints with code of conduct.
