Leads MDS Coordinator

Overview

We are seeking an experienced Lead RN MDS Coordinator to oversee the development and completion of the Resident Assessment Instrument (RAI/MDS) in compliance with state and federal regulations and facility policies. This role is a key member of the interdisciplinary team, supporting quality of care, continuity of services, and individualized, patient-centered care planning.

• Oversee the development and completion of the Resident Assessment Instrument (RAI/MDS) in compliance with state and federal regulations and facility policies.
• Support quality of care, continuity of services, and individualized, patient-centered care planning as part of the interdisciplinary team.

• Current, active RN license in New Jersey
• Proven experience as an MDS Coordinator
• Hands-on experience with MDS completion
• Strong knowledge of Medicare and Medicaid regulations and benefit guidelines
• Excellent organizational, communication, and analytical skills

• Experience in a Skilled Nursing Facility (SNF)
• Familiarity with interdisciplinary care planning and quality improvement initiatives
