Advanced Practice Nurse – Southwest NJ Team

Position: PRN.

• Conduct re-certification visits to patients on hospice in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director.
• Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on‑going aggressive treatment).
• Consult with the referring physician, the acute care team, and other members of interdisciplinary team to contribute to the patient’s plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.

Education:

• Master’s Degree in Nursing.

Professional

Experience:

• Must have a Nurse Practitioner’s license.
• 2‑4 years experience working within hospice or related field.
• Master’s Degree (listed for emphasis).
• Valid driver’s license and reliable transportation.
• Willingness to travel.
