Role OverviewProvide high-touch inpatient palliative care for patients facing serious illness, collaborating with physicians, APPs, social work, care coordination, and operational support. Work primarily Monday–Friday with flexible swap options.
Responsibilities- Perform evidence‑based, holistic, patient‑centered palliative medicine at the top of your license.
- Provide palliative care consultations and follow‑up visits across the hospital continuum, including ED, ICU, and medical, surgical, and specialty units.
- Manage complex symptoms for patients with advanced illness and chronic disease.
- Lead goals‑of‑care and advance care planning conversations with patients and families, and coordinate with SNFs or hospices when appropriate.
- Partner with interdisciplinary teams and hospital workflows, shaping the palliative medicine practice.
- Complete timely documentation aligned with clinical, billing, and quality standards.
- Focus on discharge planning and transitions of care, considering each patient’s longitudinal preferences and best interests.
- Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
- DEA + CDS registration, or ability to obtain by start date.
- 2+ years of palliative care experience.
- Certification in palliative care (e.g., ACHPN/CHPN) preferred.
- Strong communication, clinical judgment, compassion, and teamwork skills.
- 28 paid time off days in year one, increasing to 33 days at two years.
