As a Hospice RN, you will build meaningful relationships with patients and families while providing expert clinical care in the comfort of their homes. This autonomous role combines case management, patient advocacy, clinical assessment, education, and collaboration with physicians and the interdisciplinary team to ensure every patient receives compassionate, individualized care throughout their hospice journey.

Responsibilities Clinical Assessment & Patient Care- Perform comprehensive nursing assessments to evaluate patients' physical, emotional, and psychosocial needs.
- Determine patient appropriateness for hospice services.
- Develop, implement, and continuously evaluate individualized plans of care.
- Monitor patient progress and revise care plans based on changing clinical needs.
- Provide symptom management and evidence‑based nursing interventions.
- Serve as the primary Case Manager for an assigned patient caseload.
- Coordinate care with physicians, social workers, hospice aides, chaplains, therapists, and other interdisciplinary team members.
- Prioritize patient visits based on clinical needs.
- Participate in interdisciplinary team (IDT) meetings and communicate patient progress.
- Perform Home Health Aide supervisory visits in accordance with regulatory standards.
- Educate patients and caregivers regarding disease progression, medications, symptom management, and hospice philosophy.
- Adapt teaching methods to meet the unique learning needs of patients and families.
- Document education provided and evaluate understanding throughout the continuum of care.
- Complete timely and accurate clinical documentation within the electronic medical record (EMR).
- Complete admission, visit, discharge, and case management documentation in accordance with agency standards.
- Demonstrate knowledge of hospice regulations, Medicare guidelines, and insurance reimbursement requirements.
- Perform medication reconciliation during patient visits.
- Maintain current medication orders and monitor effectiveness.
- Demonstrate sound pharmacology knowledge and safe medication administration practices.
- Promote infection prevention and maintain a safe environment for patients, caregivers, and staff.
- Participate in Performance Improvement initiatives and utilization review activities.
- Assist with orientation and mentoring of new employees.
- Cross‑train to support departmental needs and share clinical knowledge.
- Attend staff meetings and continuing education opportunities.

QualificationsMinimum Qualifications
- Current New Jersey Registered Nurse (RN) License
- Minimum one (1) year of nursing experience in:
- Acute Care
- Subacute Care
- Home Health
- Hospice
- Experience using an Electronic Medical Record (EMR)
- Strong clinical assessment and critical thinking skills
- Excellent verbal and written communication skills
- Ability to work independently while managing a patient caseload
- Valid driver’s license in good standing and reliable transportation
- BSN preferred
- Previous Hospice Case Management experience
- Knowledge of Medicare Hospice Conditions of Participation
- Experience coordinating interdisciplinary care
- Strong patient and family education skills
- Experience working in community or home‑based healthcare
