Hospice RN Case Manager – $85,000–$100,000 | Mon–Fri | ~15-Patient Caseload

Location: Cherry Hill NJ
Schedule: Monday–Friday | 8:00 AM–5:00 PM
On-Call: Rotating | Limited occasional evenings/weekends
Caseload: Approximately 15 patients (subject to change based on census and acuity)

Manageable caseload (intentionally maintained around 15 patients)
* Dedicated Admissions Nurse, LPNs & On-Call Nurse support
Defined territory to reduce drive time
Structured weekday schedule
Clinician-led leadership that understands hospice

You are not expected to carry the workload alone.

As an RN Case Manager, you are the clinical lead for your assigned patients — supported by a strong interdisciplinary team.

You will:

• Conduct comprehensive nursing assessments (with Admissions Nurse support)
• Develop and manage individualized plans of care
• Collaborate with physicians, LPNs, social workers, chaplains & aides
• Educate families on symptom management and disease progression
• Participate in IDT meetings
• Maintain timely, compliant documentation

• Active New Jersey RN License
• Hospice or home health experience strongly preferred
• Strong clinical judgment & communication skills
