Position SummaryTotal Patient Care, LLC is seeking a compassionate and experienced Nurse Practitioner (APN) to join our Internal Medicine practice. The ideal candidate will provide high-quality primary care to adult patients while working collaboratively with our physicians and clinical team to promote exceptional patient outcomes.
Responsibilities- Evaluate, diagnose, and treat acute and chronic medical conditions.
- Perform annual wellness visits, routine physicals, and follow-up appointments.
- Order and interpret laboratory and diagnostic tests.
- Prescribe medications and develop individualized treatment plans.
- Educate patients on disease prevention, medications, and healthy lifestyle choices.
- Managing chronic conditions.
- Coordinate care with specialists and other healthcare providers.
- Maintain accurate documentation in the electronic medical record (EMR).
- Participate in quality initiatives and value-based care programs.
- Current New Jersey APN license.
- National Board Certification (AANP or ANCC).
- Current DEA and CDS registration.
- BLS Certification.
- Minimum of 1-2 years of experience in Internal Medicine, Primary Care, or Family Medicine preferred.
- New graduates will gain experience and mentorship from our established professionals on staff.
- Strong clinical judgment, communication, and patient education skills.
- Experienced Nurse Practitioners will benefit from our vast patient pool and knowledgeable staff of professionals serving the NJ community for over 30 years.

Base
: $115,000 - $120,000 - Paid Time Off and paid holidays
