Oncology Nurse Navigator; per-diem, day shift; Cancer Programs - Cherry Hill, NJ

The Oncology Nurse Navigator provides expert individualized care coordination to patients, and their families ensuring a timely schedule for diagnostic work-up, results reporting and communication between the patient and the multidisciplinary team ensuring the patient receives necessary referrals and follow-up. The Oncology Nurse Navigator serves as an advocate for the patient, promoting timely, competent, compassionate care and enhances the physician/patient relationship improving the experience of the patient with cancer.

* * The Oncology Nurse Navigator provides expert individualized care coordination to patients, and their families ensuring a timely schedule for diagnostic work-up, results reporting and communication between the patient and the multidisciplinary team ensuring the patient receives necessary referrals and follow-up. The Oncology Nurse Navigator serves as an advocate for the patient, promoting timely, competent, compassionate care and enhances the physician/patient relationship improving the experience of the patient with cancer.

** Job Duties
*** Collaborates and continuously partners with chronically ill or high risk patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach.
* Evaluates the needs of cancer patients and educate them about their cancer diagnosis
* Prepares patients and caregivers for what to expect throughout the cancer care continuum
* Creates and promotes adherence to a care plan, developed in coordination with the patient, PCP, oncologist and family/caregiver(s)
* Provides clinical support to Patient Navigators and Sidney Kimmel Cancer Center (SKCC) patients
* Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.  
** Minimum Qualifications
*** in Nursing
* Educate patients and families about disease process and potential side effects
* Assist patient with treatment decision making; develop and use decision aids as appropriate
* Attend and support Tumor Boards
* Reduce emergency room utilization and hospital readmissions through outpatient symptom management
* Follow patients throughout the care continuum including inpatient admissions in collaboration with inpatient care management resources
* Support transitions of care and coordination between SKCC sites
* Provide clinical consultation to the Patient Navigators
* Educate multidisciplinary colleagues about the role of the Nurseand Patient Navigators, identified barriers, available resources and the navigation process
* Support program growth and patient experience in all patient interactions

* ** Physical Demands
** Lift and carry 40-50 lbs. Examples:
Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift.  
* Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
