Social Worker III (LCSW)
The Social Worker III (LCSW) is an advanced clinical member of the Care Management Department responsible for providing comprehensive psychosocial assessment, behavioral health intervention, crisis intervention, brief therapeutic counseling, care coordination, and complex discharge planning for patients whose behavioral health, substance use, psychosocial, or other complex needs impact treatment, recovery, or transition of care.
This position serves inpatient, observation, and outpatient-in-a-bed patients requiring advanced clinical social work services within the scope of the Care Management Department, primarily for patients who do not meet criteria for management by the Emergency Department Crisis Intervention Team or inpatient Behavioral Health Unit. The Social Worker III partners with the assigned Case Manager and the interdisciplinary healthcare team to address complex behavioral health needs and facilitate individualized, patient-centered discharge planning, address complex behavioral health factors contributing to readmission risk, and promote safe transitions of care.
The Social Worker III serves as the behavioral health subject matter expert for the Care Management Department, providing clinical consultation, mentorship, education, and support to Social Worker I, Social Worker II, RN Case Managers, and the interdisciplinary healthcare team while promoting patient advocacy, regulatory compliance, and evidence-based behavioral health practice.

Qualifications:
Required education:
Master of Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program.
Required experience:
Minimum three (3) years of experience as a Licensed Clinical Social Worker (LCSW) in an acute care hospital setting.
Preferred experience:
Advanced knowledge of clinical social work practice, including psychosocial assessment, behavioral health, crisis intervention, trauma-informed care, substance use disorders, suicide risk assessment, and evidence-based therapeutic interventions in an acute care setting.
Working knowledge of Virginia Emergency Custody Order (ECO) and Temporary Detention Order (TDO) processes, Community Services Boards (CSBs), CMS Conditions of Participation, DNV accreditation standards, HIPAA, patient rights, and applicable state and federal regulations.
Demonstrated ability to perform comprehensive psychosocial assessments, develop individualized intervention and discharge plans, and manage complex behavioral health cases using sound clinical judgment, critical thinking, and crisis management skills.
Excellent communication, interpersonal, collaboration, leadership, and organizational skills with the ability to effectively manage multiple priorities in a fast-paced acute care environment.
Proficiency with electronic health records (EHR) and Microsoft Office applications.
Licenses, certifications and/or registrations:
Current, unrestricted Licensed Clinical Social Worker (LCSW) license in the Commonwealth of Virginia required.
Crisis Prevention Institute (CPI), Handle With Care, or equivalent crisis prevention/de-escalation certification preferred.
Specialized training or certification in behavioral health, substance use treatment, trauma-informed care, motivational interviewing, or other related clinical specialty preferred.
Accredited Social Worker Case Manager (ACM-SW) preferred.
