SCHEDULE:

This position will also be responsible for attending early morning, evening, and weekend events as assigned.

The Community Health Outreach Navigator serves as a frontline community-based professional responsible for connecting individuals and families to healthcare services, public benefits, food assistance programs, healthy living initiatives, and other community resources that improve quality

of life and self-sufficiency. The Navigator works to address social determinants of health by assisting community members with navigating healthcare systems, public assistance programs, food security resources, workforce opportunities, housing supports, and wellness initiatives. The

position promotes preventive health practices, chronic disease management, healthy lifestyle behaviors, and participation in community wellness programs, including incentive-based healthy living and rewards programs. The Community Health Outreach Navigator collaborates with healthcare

providers, food distribution partners, government agencies, faith-based organizations, educational institutions, and community stakeholders to improve health outcomes and reduce barriers impacting vulnerable populations throughout Hampton Roads.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Community Resource Navigation
  • Assist individuals and families in identifying and accessing healthcare, behavioral health, nutrition, housing, employment, transportation, and other essential community resources.
  • Conduct comprehensive needs assessments to identify barriers impacting health, stability, and self-sufficiency.
  • Provide resource referrals and follow-up support to ensure successful connections to services.
  • Serve as a trusted liaison between residents and community service providers.
  • Support community members in navigating complex eligibility requirements for federal, state, and local assistance programs.
Public Benefits and Healthcare Access
  • Assist residents in understanding and accessing available healthcare coverage, including Medicaid, Medicare, Marketplace plans, and other healthcare programs.
  • Educate community members regarding policy changes that may affect eligibility for healthcare and public benefits programs.
  • Conduct outreach and enrollment support activities for qualified health and assistance programs.
  • Help individuals access primary care, preventive services, specialty care, pharmacy assistance, and behavioral health resources.
  • Collaborate with healthcare partners to improve care coordination and reduce barriers to care.
Healthy Living and Wellness Incentive Programs
  • Promote healthy lifestyle behaviors through education, engagement, and community-based wellness initiatives.
  • Coordinate and support participation in healthy living reward programs, wellness challenges, fitness activities, nutrition education, and preventive health screenings.
  • Encourage participation in evidence-based programs designed to improve health outcomes and reduce chronic disease risk factors.
  • Track participant engagement and health improvement outcomes associated with wellness initiatives.
  • Facilitate workshops and educational events related to nutrition, physical activity, chronic disease prevention, and overall wellness.
Food Security and Community Food Distribution
  • Coordinate community food distribution events, food pantry operations, and mobile food outreach activities.
  • Assess food insecurity needs and connect households with food assistance resources.
  • Collaborate with food banks, grocery partners, farmers, community gardens, and hunger relief organizations.
  • Assist with enrollment in nutrition assistance programs and community-based food support initiatives.
  • Maintain accurate inventory, participant records, distribution logs, and program reports as required.
  • Support special food distribution initiatives during emergencies, extreme weather events, and community crises.
Outreach and Community Engagement
  • Develop and implement outreach strategies targeting underserved and vulnerable populations.
  • Represent HRCAP at health fairs, food distributions, community events, neighborhood meetings, and stakeholder gatherings.
  • Build relationships with healthcare systems, community organizations, schools, employers, faith-based organizations, and local government agencies.
  • Facilitate public awareness campaigns focused on health equity, food access, preventive care, and self-sufficiency.
  • Recruit participants and volunteers for community health initiatives and outreach activities.
Case Coordination and Follow-Up
  • Maintain regular contact with participants to monitor progress and address ongoing needs.
