Sentara Health is seeking a Part-time Hospice Advance Practice Provider in Chesapeake, VA. The APP will perform in-home visits, telehealth appointments, and return patient calls. Hybrid work with 24 hours per week, typically 3 days, 8am–5pm.
The NP must have a Master’s degree and relevant licensure; BLS required, ACLS preferred. Experience in Hospice/Palliative Care is valued, with various certifications listed in the posting.
