Role Overview :
Nurse Practitioner — Part Time (M/W/F), Chesapeake, VA

We are seeking a compassionate and experienced Nurse Practitioner to join our team in Chesapeake, VA on a part‑time schedule (Mondays, Wednesdays and Fridays). The successful clinician will provide comprehensive primary and acute care services for residents in post‑acute and long‑term care settings, with an emphasis on maintaining functional status, managing complex chronic conditions and delivering person‑centered care. This role requires excellent clinical judgement, strong communication skills and the ability to collaborate effectively with interdisciplinary teams.

Key Responsibilities — Nurse Practitioner, Post‑Acute Care
  • Perform comprehensive assessments, develop individualized care plans and establish measurable goals in collaboration with residents, families and facility staff
  • Diagnose and manage acute and chronic medical conditions commonly seen in post‑acute and long‑term care populations
  • Provide medication management, including reconciliation, monitoring for efficacy and adverse effects, and deprescribing when appropriate
  • Provide urgent on‑site assessment and management of clinical changes, initiating appropriate interventions and escalation as required
  • Coordinate transitions of care, including admissions, transfers and discharges, and communicate effectively with hospitals, families and community providers
  • Participate in care‑planning meetings, interdisciplinary rounds and family conferences to ensure holistic, person‑centered care
  • Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
  • Support quality and safety initiatives, including infection prevention, falls reduction and medication safety
  • Provide clinical education and consultation to nursing staff and facility teams on chronic disease management, wound care, palliative approaches and best practice
  • Maintain professional standards of practice and ensure compliance with regulatory and facility requirements
Essential Qualifications And Skills

  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution, or equivalent NP qualification
  • Current and unrestricted Virginia Nurse Practitioner license and APRN certification; valid DEA registration where required
  • Clinical experience in geriatrics, long‑term care, post‑acute care or comparable settings preferred
  • Proficiency with Electronic Medical Record systems and clinical documentation
  • Strong clinical judgement, interpersonal and communication skills; ability to work collaboratively within multidisciplinary teams
  • Commitment to trauma‑informed, person‑centered care, cultural sensitivity and ethical practice
  • Current Basic Life Support (BLS) certification required; additional certifications relevant to geriatric or acute care preferred
