Pay Range: $22.39 - $37.32 / hour

Sentara Health seeks a qualified Licensed Practical Nurse (LPN) to provide direct, one‑on‑one care to patients in their homes, monitoring health, administering treatments, and managing medications. The role collaborates with a team of RNs, physicians, and other health professionals to deliver high‑quality, compassionate care.

• Provide patient advocacy and monitor health status.
• Offer direct care, treatment, and clinical support to patients and family members.
• Administer medications and conduct nursing interventions and care plans.
• Support a holistic approach to patient care.

Education: Trade School Graduate (Required)

Licensure: LPN License active in Virginia (Required)

Certification: BLS within 90 days (Required)

Relevant nursing or caregiving experience is preferred. Experience in home health care and working with diverse patient populations is desirable.
