Licensed Practical Nurse (LPN) Case Manager
Southeastern Virginia Training Center (SEVTC) is seeking a dedicated Licensed Practical Nurse (LPN) Case Manager to provide professional nursing services and clinical case management to individuals with intellectual and developmental disabilities residing in an Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID).
The LPN Case Manager serves as a primary nursing resource and advocates for assigned individuals and works collaboratively with families, physicians, nurses, therapists, direct support professionals, QIDPs, and other members of the interdisciplinary team.
At Southeastern Virginia Training Center (SEVTC) we serve citizens with intellectual and developmental disabilities. We are an intermediate care facility operated by the Department of Behavioral Health and Developmental Services (DBHDS) where we support individuals in state-of-the-art homes with accommodations unique to their specific needs. Job duties include:
- Coordinate nursing services and manage an assigned caseload.
- Monitor health status and report significant changes or concerns to the RN and/or healthcare provider.
- Administer medications in accordance with physician orders and applicable nursing requirements.
- Monitor medication effectiveness, side effects, adverse reactions, and medication-related concerns.
- Assist with medication reconciliation following hospitalizations, appointments, and treatment changes.
- Perform nursing treatments and procedures within the LPN scope of practice.
- Provide or coordinate prescribed treatments, including wound care, catheter care, respiratory treatments, enteral feeding support, and skin care.
- Maintain accurate and timely medication, treatment, and clinical documentation.
- Provide nursing input into the development, implementation, review, and revision of nursing care plans.
- Participate in Individual Support Plan (ISP) meetings and annual or significant-change reviews as assigned.
- Collaborate with the interdisciplinary team regarding health-related components of the ISP.
- Monitor progress toward health-related outcomes and recommend appropriate nursing interventions.
- Participate in medication reviews, medication-error prevention activities, quality assurance activities, and laboratory audits.
- Serve as a member of the Pharmacy and Therapeutics Committee and function as a Medication Variance Reviewer as assigned.
- Participate in required meetings, training, committees, drills, and continuing education.
Skills and experience required include:
- Knowledge of practical nursing principles and practices.
- Strong organizational, communication, documentation, and interpersonal skills.
- Ability to establish priorities and effectively manage an assigned caseload.
- Ability to perform nursing tasks and functions independently within the LPN scope of practice.
- Basic computer and electronic health record skills.
- Current and valid license as a Licensed Practical Nurse in Virginia or a participating Nurse Licensure Compact state.
- Ability to successfully complete required pre-service training, including medication administration, CPR, First Aid, and state-approved behavior interaction procedures.
- Valid driver's license.
Bonus points if you have:
- One or more years of experience working with individuals with severe intellectual and developmental disabilities preferred.
- Previous case management experience preferred.
