About the Position
Immediate opportunity in Virginia to join a network of Community-based primary care clinics to help serve an underved population. This is a rewarding opportuntiy for experienced providers that enjoy being an integral part of the community. Full time, permanent position with a M-F Days schedule with shared telephone call. Providers will have their own panel. AGNPs welcome to join as well.

The team are looking for candidates that are proficient with EMR systems and computer savvy.

Experience with chronic disease management a plus.
