Physical Therapist (PT) – Part Time
Cheshire, CT – Cheshire Civita Care Center – $45–$52/hour | Part-Time
Post Rehab is adding a part‑time Physical Therapist to its rehab team in Cheshire. This role will support resident rehabilitation and mobility goals. This role offers the opportunity to work with a diverse caseload while making a direct impact on patient independence and quality of life.
What You’ll Do:- Complete evaluations and establish functional therapy plans
- Provide hands‑on treatment to improve strength, balance, and mobility
- Adjust interventions based on progress and clinical judgement
- Document care and collaborate with the interdisciplinary team
- Active Physical Therapist license
- Interest in geriatric and post‑acute care
- Prior SNF experience is helpful but not required
