Job Title: Admissions Director for SNF

Civita Care Center at Cheshire is seeking a highly skilled and compassionate Admissions Director for our skilled nursing facility (SNF) in Cheshire, Connecticut.

• Coordinate and facilitate the admission process for patients transferring from hospitals to our SNF, ensuring seamless communication with healthcare providers and families

• Conduct thorough assessments of patients' needs, including medical history, treatment plans, and social services requirements

• Develop and maintain relationships with local hospitals, physicians, and other healthcare partners to promote our services and build strong referral networks

• Collaborate with our interdisciplinary care team to create personalized care plans that meet each patient's unique needs and goals

• Monitor and report on patient outcomes, identifying areas for improvement and implementing process changes as needed

• Participate in performance improvement initiatives and quality assessment activities

• Maintain accurate and detailed records, adhering to regulatory standards and facility policies

• Bachelor's degree in a related field (e.g., healthcare administration, nursing, social work)

• Minimum of 2 years of experience in healthcare administration, admissions, or a related field

• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals

• Ability to work in a fast-paced environment, prioritizing tasks and managing competing demands

• Familiarity with medical terminology, insurance policies, and healthcare regulations
