Post Rehabilitation Services is hiring a Physical Therapist at Civita Care Center 'll help residents regain mobility after illness or injury and address ongoing physical challenges through care tailored to their daily needs.
What You’ll Do- Evaluate residents' movement and establish functional treatment goals
- Provide therapy to improve strength, endurance, balance, and walking
- Address transfer safety and recommend appropriate mobility devices
- Reassess residents and update care plans as their abilities change
- Document outcomes and coordinate recommendations with nursing and the rehab team
- Medical, dental, and vision insurance; 401(k)
- CEU reimbursement, parental leave, and disability insurance
- Unlimited referral bonus, with up to $1,000 per successful referral
- Support from the therapy team and regional leadership
- Active Connecticut Physical Therapist license
- Strong assessment, communication, and documentation skills
