Your Role as a Social Work Director:

As the Social Work Director at Civita Care Center at Cheshire, you will play a pivotal role in providing emotional support, counseling, and advocacy for our residents and their families. Responsibilities:

As our Social Work Director, you will be responsible for:

• Providing emotional support and counseling to residents and their families
• Conducting psychosocial assessments and developing care plans
• Advocating for residents' rights and needs
• Collaborating with the interdisciplinary care team to enhance resident well-being
• Ensuring compliance with regulatory requirements

Qualifications:

To be considered for this role, you should meet the following criteria:

• Prior SNF experience.
• Possession of a valid state Social Work license
• Previous experience in social work or related field preferred
• Excellent communication and interpersonal skills
• Empathy and a commitment to resident well-being
