Penn Home Health Care is seeking a compassionate Medical Social Worker (MSW) to support patients and families receiving home health services.
Service Areas:
Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, PA
Position Type:
Per Visit / PRN
Responsibilities:
• Conduct psychosocial assessments.
• Identify community resources and support services.
• Assist patients and families with long-term planning and coping strategies.
• Coordinate referrals and community services.
• Participate in interdisciplinary care planning.
• Complete required documentation.
Qualifications:
• Master's Degree in Social Work from an accredited program.
• Current Pennsylvania licensure if required by applicable regulations.
• Experience in healthcare, home health, hospice, or related setting preferred.
• Valid driver's license and reliable transportation.
