Dutiesand Responsibilities
Provide nursing care in accordance with the patient's plan of care, including comprehensive health and psychosocial evaluations.
Monitor the patient's condition, promote health and prevention, coordinate services, and deliver direct nursing care.
Conduct teaching and training activities for patients and their families.
Job ConditionsMust possess a valid driver's license and be willing and able to drive to patients' residences.
Must be able to access patients' homes, which may not be routinely wheelchair accessible.
Sufficient hearing, eyesight, and physical dexterity are required to perform physical assessments and patient care tasks.
Physical activities include walking, sitting, stooping, standing, and lifting patients (minimal to maximum) and turning patients.
Must communicate effectively in English, both verbally and in writing, as frequent communication via telephone and written reports is necessary.
QualificationsGraduation from an accredited School of Nursing.
Current license to practice as a Registered Nurse in Pennsylvania.
One or more years of experience in a community/home health agency or hospital setting is preferred.
Knowledge of Medicare and Medicaid guidelines.
Working knowledge of home health care principles, techniques of professional nursing, and related documentation requirements.
Proficient organizational and time management skills, along with an understanding of management processes.
Ability to contribute to the quality of care through constructive communication with nursing managers and staff.
