Home Health Nurse

Chestertown, MD, 21620 (Kent & Queen Anne's Counties, Maryland)

6 months with possible contract extension

• Shift: 8:00 AM – 4:30 PM
• Weekly Hours Guarantee: 40 hours

We are seeking an experienced Registered Nurse (RN) to provide comprehensive home health care services across Kent and Queen Anne's counties. The RN will manage patient care episodes, complete OASIS documentation, and ensure high-quality coordinated care for home health patients.

• Provide skilled nursing care in accordance with physician orders and patient care plans
• Manage a caseload of home health patients, ensuring continuity and quality of care
• Perform comprehensive patient assessments and monitor changes in condition

• Complete and submit OASIS documentation for admissions, discharges, and resumption of care
• Conduct routine visits (RV) and ensure accurate clinical documentation

• Collaborate with physicians, therapists, and interdisciplinary team members
• Communicate patient progress, concerns, and care plan updates effectively
• Coordinate services to ensure optimal patient outcomes

• Maintain accurate and timely documentation in compliance with regulatory and payer requirements
• Adhere to infection control protocols and agency policies
• Ensure all clinical activities meet state and federal home health standards

• Area:
  Kent County, MD
• Queen Anne's County, MD

• Active Registered Nurse (RN) license
• Home health experience (mandatory)
• Strong knowledge of OASIS documentation
• Ability to work independently and manage multiple patients

• Experience with Homecare Homebase (HCB)
• Strong case management and organizational skills

• Excellent communication and patient education abilities
