- Coordinates and supervises the provision of respiratory care and activities of The Respiratory Care Services Department and clinical areas of designated responsibility. Assures compliance with the directives, policies, and applicable laws and regulations.
- Lead and oversee daily operations of the Pulmonary Rehabilitation (PR) program, including staff training and performance management.
- Review patient charts to process referrals, ensuring eligibility based on PFTs, EKGs, imaging, and insurance verification.
- Coordinate intake appointments and exercise session scheduling for new PR patients.
- Collaborate with Pulmonary Physicians to onboard patients through comprehensive intake evaluations, including 6‑minute
- Develop individualized treatment plans (ITPs) for each patient, incorporating goals across five core areas:
Exercise, Nutrition, Psychosocial Support, Oxygen Therapy, and Additional Needs. - Ensure ITPs are regularly updated and that patients receive face‑to‑face evaluations with Pulmonary Physicians every 30 days to meet regulatory requirements and monitor goal progress.
- Coordinate patient scheduling with the Registration Team.
- Conduct discharge and graduation appointments, ensuring all documentation is scanned into Epic upon program completion.
