Make an ImpactArkos Health seeks a Nurse Practitioner with strong clinical skills to care for members with acute and chronic health conditions. Responsibilities include assessment, diagnosis, treatment planning, health promotion, education, and care coordination. This role is recognized as an Advanced Practice Registered Nurse and Independent Licensed Practitioner by the Wyoming State Board of Nursing, with practice aligned to state laws and regulations.

- Conduct comprehensive in‑home history and physical assessments for members, tailored to age, gender, and clinical needs. Visits may occur in homes, facilities, or provider offices and encompass various assessment types including acute, palliative, follow‑up, CHA, or home health evaluations.
- Utilize expertise in acute and chronic conditions to assess risk factors, etiologies, and pathophysiology.
- Review member records to accurately capture diagnoses, interpret diagnostic studies, and formulate care plans using critical thinking.
- Design, implement, evaluate, and adjust treatment plans as required.
- Recommend and modify medications when necessary.
- Document all care accurately and promptly in the electronic health record, including correct diagnosis codes, status of health conditions, and plans for each diagnosis.
- Use correct diagnosis codes.
- Update status of acute and chronic conditions.
- Active Wyoming RN license
- Bachelor’s in Nursing and Wyoming Advanced Practice Nursing license (Master’s preferred)
- National Board Certification as Nurse Practitioner
- DEA licensure with unrestricted prescribing privileges
- At least 1 year of experience in ambulatory or community care, or relevant clinical experience with strong assessment skills
- Experience with adult, older adult, or geriatric patients preferred
- Population health management experience preferred
- Strong problem‑solving and prioritization skills for dynamic patient needs
- Adaptable and flexible with ability to adjust to changing organizational priorities
- Driving required; must have valid driver’s license and proof of insurance
