• Provide skilled nursing services to patients in their homes, including medication management, wound care, and treatment of chronic conditions
• Conduct thorough assessments and develop individualized care plans to meet each patient’s unique needs
• Collaborate with patients, families, and interdisciplinary care teams to ensure seamless care coordination
• Monitor patient progress, identifying and addressing potential issues to optimize health outcomes
• Maintain accurate and detailed records of patient care and treatment

• Current RN license in the state of Wyoming

• Ability to work independently and as part of a team
• Strong communication and interpersonal skills
• Physical stamina to lift, push, and pull equipment and supplies
