The Infusion Nurse provides primary patient care for assigned patients, including assessments, care planning, evaluation, and education for patients receiving infusion therapy and related nursing services in accordance with care matrices and best practices in the home or alternate site setting.

• Provide direct care for all assigned patients, initiating and maintaining communications with pharmacists, prescribers, dietitians, home patient representatives, care partners, and community resource staff.
• Evaluate and educate patients or care partners to safely administer home infusion therapy and provide additional instruction as needed.
• Evaluate the home environment and identify interventions required to ensure safety.
• Develop, with team members, the plan of care and evaluate response to therapy with each encounter.
• Manage schedule via company‑approved platform per company policy, communicating any changes promptly.
• Coordinate patient care via the approved platform.
• Participate in the nurse on‑call rotation for continuous nursing support.
• Document all encounters, communications, and care activities at the time of service as required by policy and regulations.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment and required clinical documents per policy and state guideline.
• Work with departments to ensure patient documents (consents, plans of treatment) are completed and returned to the branch office in a timely manner.
• Perform other duties as assigned.

• Per diem, 20–25 hours per week, varying schedule
• Must participate in an on‑call rotation
• Must be knowledgeable of IV access devices (IV lines, ports, infusion medications)
• Must drive up to 1 hour to see patients; flexibility for up to 2 hours as needed (mileage reimbursement and pay for driving)
• Must be licensed as an RN in Wyoming

• Current RN license in good standing
• BSN preferred
• Advanced certification in specialty areas (CRNI, ONS, IgRN) preferred but not required
• Participation in on‑call rotation, nights and weekends included
• Administrative oversight responsibilities
• Emergency availability during nights, weekends, holidays as required
• Minimum 3 years nursing experience with infusion therapy preferred
• Experience in ICU or cardiac care preferred
• Valid state‑issued driver’s license with no significant restrictions
• Proficient with Microsoft Outlook and Office Suite
