Physician to join our growing team. We have physician opportunities available across four locations in the Chicago area:
Cicero (60802), Chicago (60631), Chicago (60641) and Chicago (60659).

• $250,000–$400,000 base salary

• Build strong,lastingrelationshipswithpatientsandfamilies.
• Collaborate with Advanced Practice Providers ,Care Managers,Behavioral Health professionals,andspecialtyteams.
• Coordinate referrals,diagnostic services,andfollow-upcare.
• Maintainaccurateandtimelyelectronichealthrecord(EHR) documentation.
• Follow applicable clinical, regulatory, and quality standards while supporting value-based care initiatives.

• MDorDOdegreefromanaccreditedmedicalschool.
• Board Certifiedor Board Eligible in :
• Family Medicine,
  or
• Internal Medicine-Pediatrics(Med-Peds).
• Active Illinoismedicallicenseorabilitytoobtainonebeforestarting.
• DEAregistrationandapplicablecontrolledsubstanceauthorization,orabilitytoobtainthemasrequired.
• Strong commitment to patient-centered, high-quality care.

• Spanishor Polishfluencyispreferredbutnotrequired.
