The Role
The School Transition and Placement Coordinator plays a vital role in supporting students as they move from HPDS to schools in their home communities. This position involves collaborating with families, educators, and service providers to ensure each student's transition is as seamless and successful as possible. You will assess individual student needs, identify appropriate school placements, coordinate necessary supports, and provide follow-up services after students have transitioned.
In addition, you will plan and manage graduation activities, from ceremonies to yearbook production, helping our students and families celebrate their achievements.
- Coordinating educational placements for students transitioning from HPDS.
- Meeting with parents, teachers, and service providers to ensure alignment with each student's learning needs.
- Leading and supporting the IEP process for transitioning students.
- Organizing and overseeing graduation events and related activities.
- Maintaining ongoing professional development and sharing best practices with colleagues.
- Representing HPDS at meetings with partner schools and in the broader educational community.
- A bachelor's degree or higher in learning disabilities, special education, or social work (Master's preferred).
- Proven experience placing students with learning disabilities in appropriate educational settings.
- Knowledge of public and private schools within the Chicago metropolitan area.
- At least three years’ experience working with children with diagnosed learning disabilities (preferred).
- Strong organizational, problem-solving, and communication skills.
- The ability to work effectively and efficiently while managing multiple priorities.

Salary: $35,000 - $40,560 per month.
