The House Manager has overall responsibility for clients living in the residential program during their shift. They will provide direct clinical services to assist with independent living skills, crisis management, and medication compliance observation. The House Manager will communicate essential client information to their Supervisor, Providers, medical staff, and other residential employees during their shift and at shift change. They will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders using a harm reduction approach.
Responsibilities- Work directly with clients regarding case management needs, training in independent living skills, and provide individual/group services or crisis intervention as needed.
- Provide recovery-focused clinical services to residential clients to assist with independent living (i.e., personal hygiene, housekeeping skills, room inspections, nutrition, cooking, grocery shopping, medication monitoring, etc.), with the goal of living in the least restrictive environment for each person.
- Model good communication skills, recovery-oriented living skills, effective coping skills, and self-help strategies for clients.
- Collaborate with team members, prescribers, and outreach treatment teams to deliver quality service and effective interventions on behalf of the clients.
- Assist clients in identifying signs and symptoms of decompensation and work collaboratively to develop coping skills to manage symptoms and stressors.
- Provide psychoeducation, medication training, and monitoring to clients according to Trilogy policy.
- Coordinate with team nurse/pharmacy to ensure client medication accuracy.
- Educate and assist clients with applying for entitlements (e.g., Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK).
- Act as liaison for clients, Trilogy, and housing provider to facilitate long-term housing stability and safety.
- Partner with transition teams to review and admit clients into the Residential program and oversee the safe transition of the client into the community.
- Recognize opportunities to move clients to appropriate levels of care and provide referrals to additional services as indicated.
- Maintain documentation and charts to ensure compliance with Medicaid, CARF, CILA, funding bodies, and organization standards (e.g., notes, medication logs, safety drills).
- Complete daily progress notes based on services provided to clients in a timely manner, reflecting Trilogy’s documentation policy and agency standards.
- Complete all necessary documentation related to client care (e.g., IMCANS, LOCUS, tracking forms in the Electronic Medical Record).
- Understand the representative payeeship process per agency policy and provide individualized client money management services.
- Advocate on the client’s behalf and empower clients to advocate for themselves when appropriate.
- Communicate effectively within the team model for a multi-disciplinary approach to client care.
- Actively participate in team communication via multiple electronic platforms.
- Engage regularly in weekly team meetings to consult and collaborate on shared caseload.
- Attend monthly department meetings for training and important updates.
- Plan and facilitate special activities and outings for the clients.
- Perform other related duties and/or projects as assigned.
Must have one of the following requirements:
- High School Diploma or GED and two years of supervised clinical experience in a mental health setting.
- Associate’s or Bachelor’s degree in any field.
- CPR/Adult First Aid/AED certification.
- Fingerprint Background Check.
