LSSI is hiring a full‑time Counselor Case Manager, Tuesday through Saturday 8:30 AM to 5:00 PM. The role is responsible for providing individual, group, and family counseling in a community‑based setting to primarily adolescent clients experiencing substance use disorders (SUDs) and related issues. The counselor will work in a clinically and culturally sensitive manner within the Substance Use Outpatient Treatment Program, helping clients achieve their goals, and partnering with internal and external treatment professionals and community partners.

Responsibilities also include completing IM+CANS, treatment plans, and other required documentation.

Essential Functions
  • Provide a continuum of counseling services to individuals with alcohol/drug‑related conditions and/or problems using a variety of therapeutic modalities and evidence‑based practices.
  • Coordinate and facilitate group and individual counseling sessions (ASAM Levels 0.5, I, II.1, continuing care, DUI), and psychoeducation in community‑based settings.
  • Maintain an assigned caseload and associated documentation, including assessment, treatment planning, continuing care, case management, and discharge plans; monitor progress and participants’ response to treatment.
  • Communicate with referral sources to provide status reports and completion/discharge documentation.
  • Maintain weekly performance productivity.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, and collaborative organizations with dignity, respect, honesty, and fairness.
Position Qualifications
  • High school diploma or equivalent required.
  • Associate’s, bachelor’s or master’s degree in social work, psychology, counseling, or related field preferred.
  • One (1) to three (3) years of experience in the substance use field preferred.
  • LSW, LPC, LCSW, LCPC or CADC required, or must obtain within two (2) years of hire date.
  • Proficient speaking, reading, and writing Spanish preferred.
  • Annual TB test and biohazard training required.
  • Experience working with adolescents preferred.
  • Knowledge and experience using DSM‑5 and ASAM criteria to make accurate diagnoses of substance use disorders and treatment/intervention recommendations required.
  • Proficiency in EHR documentation and review preferred.
  • Experience establishing and maintaining collaborative community partnerships preferred.
  • Experience using Zoom or other telehealth platform preferred.
  • Excellent oral and written communication skills.
  • Strong organizational, presentation, and computer skills, including experience with Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) and related software.
  • Background check clearance required.
  • Valid driver’s license, in good standing for the state of residency preferred.
  • Access to reliable transportation preferred.
  • Valid Illinois statutory minimum liability insurance coverage, bodily injury and property damage preferred.
