Position Summary

Provide intensive, individually tailored, strength-based, and collaborative case planning services to high risk families referred through Erie County Department of Social Services

Compensation

Bachelor's Degree: $21.30 - $23.38 per hour
Master's Degree: $23.10 - $25.18 per hour

Hours/Schedule

Full-Time
Monday - Friday, with non‑traditional hours as needed to best support the needs of the program

Position

Specific Duties /Responsibilities
  • Quickly engage families in a face‑to‑face in‑home contact to identify precipitating safety & risk issues, family strengths/needs to develop a safety plan and establish a comprehensive service plan to achieve treatment outcomes.
  • Work intensely with referral sources, community service providers, and all family members to ensure collaborative efforts toward established service plan. Build upon support networks for each family and ensure adequate and stable supports are in place to stabilize families, address immediate needs and minimize safety and risk concerns.
  • Maintain intensive in‑home contact, service provision, and therapeutic rapport with families which includes face‑to‑face support, education, advocacy, case management, and crisis intervention services, for a minimum of 10‑15 hours per week per family.
  • Ensure accurate, thorough, and timely completion and filing of all required documentation in CONNECTIONS according to contractual and agency requirements.
  • Develop, teach, model and evaluate effective, culturally competent, trauma informed techniques and interventions with parents/caretakers and children/youth.
  • Ensure all court mandated and/or recommended services are offered, utilized, and maintained via referral/linkage or provided directly to minimize risk, while enhancing the family’s safety, permanency, and overall well‑being.
  • Actively participate in required/offered meetings and training opportunities.
  • Other duties as assigned.
Knowledge, Skills, and Abilities
  • Must have excellent communication and customer service skills.
  • Ability to multi‑task and be detail oriented.
  • Ability to work independently and be an enthusiastic team player.
  • Must have a high degree of discretion dealing with confidential information.
  • Available to work non‑traditional hours, including weekend and evenings to support specific programmatic schedules, crisis intervention needs, and on‑call support.
  • Ability to provide frequent transportation of children and families.
  • Ability to work effectively with clients, staff, families, and community contacts.
Qualifications
  • Bachelor’s Degree in a human services related field with 4 years of experience.
  • Master’s Degree in a human services related field.
  • Valid New York State driver’s license, adequate auto insurance, and reliable transportation to travel locally on a regular basis is required.
