PSYCHIATRIC REHABILITATION SERVICE COORDINATOR PRSC - PARK VIEW REHAB CENTER

Park View Rehab Center is a skilled nursing facility seeking a Psychiatric Rehabilitation Service Coordinator (PRSC) to whom the resident primarily relates for the coordination of services, programs, oversight of data collection and review of resident performance.

• One year working with the adult mentally ill population; preferred but not required.
• Bachelor's Degree in Human Services field (including sociology, social work, special education, and psychology)
• Experience with caseloads, crisis intervention, documentation, care planning with involving families in the process, knowledge of the MDS, discharge planning, facilitating skills groups and psycho-social groups and problem solving
• Good assessment skills, interpersonal skills, and knowledge of MDS 3.0 along with basic computer skills, communication, advocacy skills and charting experience
• Must be able to work in an interdisciplinary team environment
• Must be able to handle confidential data with professional discretion
• Position is 5 days/weeks 8 hours per day.
