Metropolitan Family Services is a Chicago‑based agency providing comprehensive human services since 1857. We are seeking a full‑time Care Coordinator for our Southeast Chicago location at 3062 E 91st St, Chicago, IL 60617, USA.
SALARY: The average starting salary for this position falls in the range of $47,000 to $52,000 annually, based on skill and experience.
ESSENTIAL JOB FUNCTIONS:- Provides outreach and engagement services to assist in linkage to appropriate integrated services based on acuity level and/or response to a populating care task.
- Monitors IHPA population health management platform (Health
EC) to complete care tasks that populate for members and identify members with high utilization of ER visits and hospital admissions so targeted interventions can be provided. - Provides education and training to MFS staff and other community providers (e.g., hospitals, Federally Qualified Health Centers) regarding care coordination, its purpose, IHPA workflows and processes.
- Monitors Admission Discharge Transfer (ADT) feed daily for IHPA members entering/discharging from an emergency room, inpatient hospitalization, or other coordinated care interface, following up with members at the ER and/or hospital, and facilitating post‑discharge appointments.
- Documents the linkage and follow‑up care, as well as other vital information in both MFS electronic health record and IHPA platform after each contact and service.
- Participates as appropriate in IHPA member education, including development of materials, conducting presentations, or supporting MFS staff in such efforts.
- Works with multidisciplinary professionals, including Managed Care Organization (MCO) Care Coordinators, to provide a continuum of coordinated care to meet the complex needs of members.
- Maintains timely and accurate client records and any necessary agency and IHPA required program reports.
- Participates in required MFS and IHPA trainings and meetings.
- Other duties as assigned.
- Excellent oral and written communication skills, including ability to use social media, provide trainings and community presentations, and proficiency in Microsoft Office Suite and Google tools.
- Punctual, reliable, and dependable with the ability to work flexible hours (some evenings and weekends).
- Ability to work effectively with diverse individuals and groups.
- Self‑starter that is detail‑oriented and thinks creatively and strategically.
- Ability to produce high‑quality work independently.
- Demonstrated commitment to social justice, community empowerment, and advocacy.
- One year of experience providing case management and/or community support services in the behavioral health field (required).
- Bachelor’s degree in Psychology, Social Work, or Human Services related field (required).
- Experience working with diverse clientele with behavioral health needs (preferred).
- Knowledge of and/or experience with care coordination and Managed Care Organizations (MCO) (preferred).
- Satisfactorily pass all relevant screenings and criminal background checks.
- Driving for work required with transportation of clients in a personally owned vehicle.
- While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform activities such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
