Social Service Coordinator – Evergreen Real Estate Group
Location:
Chicago, IL
• Salary: $65,000.00–$80,000.00

Key responsibilities include coordinating home and community based services, providing case management, crisis interventions, community referrals, program coordination, and maintaining resident files per HUD requirements.
Essential Duties and Responsibilities- Assess residents who may require assistance in daily living activities or a higher level of care.
- Make referrals to community service agencies and follow up on services.
- Coordinate with hospital staff regarding discharge planning for residents returning to their apartments.
- Provide crisis intervention with residents on a short‑term basis.
- Provide case management services for residents receiving services from various community agencies.
- Advocate for residents and maintain confidentiality.
- Maintain current knowledge base of services and programs impacting seniors and/or disabled residents to enhance assistance.
- Identify education and health programs to improve residents’ quality of life.
- Establish linkages and maintain contacts with community agencies offering services to residents.
- Develop and update resource directory.
- Build an informal support system for residents with family members, other residents, friends, etc. as needed.
- Publish and distribute a building newsletter.
- Fundraise to cover the cost of programs and activities in the building.
- Demonstrate sensitivity and responsiveness to cultural differences within the resident population.
- Maintain resident confidentiality.
- Utilize social services software to maintain resident files in compliance with HUD requirements.
- Report instances of abuse or neglect and take proactive steps to protect residents.
- Participate in overall planning for activities at the property.
- Serve as a resource to the Manager related to ongoing policy development and social planning within the building.
- Communicate and coordinate with property management staff regarding program and resident issues.
- Participate in professional organizations and training opportunities to meet HUD CEU requirements.
Must be punctual and meet performance, attendance standards, work deadlines, and scheduled breaks.

- External Working Relationships – Develops and maintains courteous and effective working relationships with clients, vendors, and other representatives of external organizations.
- Cost Consciousness – Works within approved budget; develops and implements cost‑saving measures.
- Safety and Security – All employees are responsible for observing safety and security procedures as applicable and reporting potentially unsafe conditions to management.
- Effectively communicate with residents (handle concerns, complaints and issues); community organizations and providers; and other staff.
