Job DescriptionThe Transplant Nurse Coordinator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory and accreditation standards. A registered nurse with experience in stem‑cell or organ transplantation and donation, the coordinator practices under the supervision of transplant physicians and provides coordination through all phases of patient care, including referrals to external clinicians, treatment programs and community resources as appropriate.
Responsibilities- Plan, direct, provide, and document appropriate care for patients in the designated setting.
- Assist in coordinating comprehensive care, including the consent process, data collection, and arranging follow‑up visits, referrals, testing, treatments, and long‑term follow‑up.
- Work collaboratively with the interdisciplinary team to coordinate ongoing care, focusing on cost‑effective ways to maximize wellness and enhance patients’ lives.
- Develop, coordinate, and provide education for patients, families, caregivers, and staff based on personalized needs, serving as a resource for ongoing education.
- Serve as a communication link between the patient, family, caregivers, interdisciplinary team, and outside hospitals/agencies.
- Contribute to departmental, hospital or other professional committees, and quality or process‑improvement initiatives.
- Ensure clinical practice and patient care are consistent with policies, procedures, and regulatory requirements.
- Perform other duties as assigned.
- Bachelor of Science in Nursing (BSN).
- Licensed Registered Nurse (RN) in the state of Illinois.
- CPR‑Basic Life Support (BLS).
- Minimum 2–3 years of recent related clinical experience.
- Specialty certification in the clinical area of focus.
- Previous transplant or organ‑specific experience (e.g., hepatology, nephrology, hematologic malignancies, hematopoietic stem cell transplant, cellular therapies).
